Daniell Summers, NP, and Dr. David Nibel from Daviess Martin Medical Clinic will be at Shoals High School on Wednesday, April 3, offering FREE sports physicals for Shoals students in grades 4-11.

⏰ TIME: 10:00 a.m. – 5:00 p.m.

💰 COST: FREE for all Shoals students!
📄 WHO: Shoals students in grades 4-11
IMPORTANT DETAILS:
✔ A parent does NOT need to be present as long as the required physical paperwork is filled out and signed.
✔ Physicals completed after April 1 are valid for summer 2025 sports AND the 2025-2026 school year.
✔ Be prepared—bring your completed and signed physical form.
