Perinatal Pathways at DCH

Daviess Community Hospital is committed to reducing high infant mortality rates by providing comprehensive care and support for expectant mothers, infants, and families. Through our Perinatal Pathways program, we offer confidential screenings, expert guidance, referrals, treatment, and education. Our goal is to ensure every family has the resources they need for a healthy pregnancy, birth, and postpartum experience.

Meet Our Perinatal Navigator

Our Perinatal Pathways team is led by Desireé Lewis, BSN, RN, CHC, a nurse with more than 15 years of maternal health experience. As our dedicated Perinatal Navigator, Desireé specializes in supporting women with medical conditions during pregnancy and the postpartum period. She also assists patients who face barriers to accessing healthcare—particularly those without insurance coverage—in receiving prenatal and child well visits.

How We Help

By working with our Perinatal Navigators and support staff, families receive tailored guidance that connects them to crucial healthcare resources and services, including:

  • Assistance in obtaining prenatal care
  • Linking families to community support programs
  • Education on healthy pregnancy and infant care
  • Financial and social service referrals

High-Risk Pregnancy Care

If your OB-GYN identifies any health concerns that classify your pregnancy as high-risk, Perinatal Pathways can help. Our dedicated Perinatal Navigator works closely with you throughout your pregnancy to:

  • Provide comprehensive education on your specific health needs
  • Offer guidance on managing medical conditions
  • Coordinate care among specialists
  • Advocate for a safe and positive birth experience
